Report A Loss
Controlled Substances
Please, fill out and submit the CS Reportable Discrepancy Report: Form 5 (pdf) for the theft or loss of any amount of a controlled substance to the following:
- Emory Police Department (EPD)

- Research Controlled Drug Compliance (RCDC)
- Email – rcdc@emory.edu
Thefts or losses of any amount of a controlled substance must be reported to the Emory Police, RCDC, Drug Enforcement Agency (DEA), Georgia Pharmacy Board (GPB), and Georgia Drugs and Narcotics Agency (GDNA).
Researchers are advised to contact RCDC for assistance in completing reports to DEA, GBP, and GDNA before submission.
- DEA- Complete and submit DEA Form 106 online within one business day of discovery.
- GDNA- Within 48 hours of discovery, fax a copy of DEA Form 106 to (404) 651-8210. For questions, contact them at (404) 656-5100.
- GBP- Within 48 hours of discovery, fax a copy of the DEA Form 106 to (470) 386-6137.
Please complete and save this standard operating procedure in your records (due diligence security process).
Dangerous Drugs
Please, fill out and submit the Dangerous Drugs Discrepancy Report: Form E (DOCX) for the theft or loss of any amount of dangerous drugs to the following:
